Ask HN: Is Touch ID fail-proof?

Are reads accurate 100% of the time (a given fingerprint producing the same ID for all reads)? Is there a chance for collissions (2 different fingerprints producing the same ID)? Can it be physically hacked (for example placing a picture of a fingerprint)?
