How do you keep your project wiki or confluence up to date?

I returned to a confluence project after a few months away, and Im uncertain how up to date or relevent the information is. I know the project has changed, but am unsure how much the wiki has been maintained. What strategies do people use to keep this information up to date? Especially for small teams
