IT Strategies with Context and Results?

Does anyone here have a story of a discovery they or others made, which led to IT improving the their efficiency/performance? I'd love to here some short stories along the lines, "..was working in industry, noticed something odd, investigated, ended up proposing solution, and it worked."

I'd also be interested in the opposite. When was an opportunity recognized but for different reasons it wasn't possible to gain the efficiency expected.
