Ask HN: How does your company manage its policies?

A startup I recently worked for (in customer service / operations) changed its policies frequently (often weekly and sometimes daily). It was tough to keep my team (not to mention customers) informed on the latest policies.

Although there was a knowledge base that was kept almost up-to-date, it was usually incomplete and, more surprisingly, it didn't seem possible to 'revert' policies. I.e., if a policy didn't work, it would be changed, but there was no record (AFAIK) of policy versions or what had been tested and failed.

Does your company experience this challenge? How does it handle it?
