The logistics of buying phone numbers in multiple countries, dealing with all their legal limits on how you can use phones, and legally hooking all of the magic Internet-to-Phone-System hardware has to be a bit prohibitory for international release.
Skype manages it.
Cannot imagine why the comment "Skype manages it" would be killed. It's 1- true, and 2- a good point. If Skype/Ebay can do it, why not Google? If not now, I'd assume it's coming.
Yeah, Skype does manage it. Did Skype manage it within 24 hours of the initial release of the service? Skype started out as internet only, then US only phone, then international phone. Yeah I expect it is coming, and the fact Skype can do it is a good point.
I'm pretty sure computer-to-computer was available globally from the beginning, and the oldest SkypeOut review I could find during a quick search involved someone calling the US, Canada, Romania and Argentina.

Skype was a European company, of course, and the headquarter and engineering office is still there. A slightly different perspective, perhaps?
