Google Chrome's Block Site Auto Installing?

Fired up my MacBook Pro that was sleeping since last night, and Chrome showed a "Thank you for installed Block Site" which I very much did not do.

Anyone else experience this?

I uninstalled it and was present with a Google Form asking why. I gave the reason "I didn't ask for this to be installed"
