Major website dumpster fires

I'm booked on a cruise this coming May, and Royal Caribbean just rolled out a completely broken update to their site (to the point where you can't even access your reservations). What other companies have you seen completely trash a live website? Have you ever worked for a company that does so little testing as to allow things like this to happen? I'm curious to hear other examples of this type of thing and views of it from behind the scenes...
