Ask HN: How to regulate policies on media/posts?

I'm curious how this is done. I know that some are developing ML techniques to identify potentially inappropriate content. Otherwise how is this regulated?

On big social media sites do they have a gigantic staff that is responsible for reviewing content that is reported? Or is it based on the percentage of people who saw and reported something?
