Ask HN: What is the biggest limitation of having an artificial heart today?

I have read that blood flow is not smooth as a real one. Do you think is that a hard limitation to solve or there are new approaches upcoming? Or is there a bigger problem than flow itself? (Excluding the wear out and failure issues)
