IP assignment in software development contracts

I looked into different software development agreements and my question is about the (work for hire) clause as I read that it might not work in some cases. opinions are to add an Intellectual Property assignment clause in case the (work for hire) didn't work. I was wondering if we shall include an IP assignment or an irrevocable exclusive license worldwide, which is better? can we include both. can anyone provide sample clause or contract?
