Ask HN: Open-source solutions for static petition sites

I have a static website running on HuGo hosted in GitLab. I would like to provide a form to visitors to showcase their support on the cause. Just a Name (optional Surname) and an email (for confirmation and optionally a link to more info) would be more than enough.

I've seen FSFE's publiccode.eu HuGo based website doing so but I'm anaware of any other solution.

Are you aware of any open-source solutions to do such a thing?
