Ask HN: How often do you do usability testing for your product?

After talking to several product owners and asking this question I was shocked with the answers. Some of them performed usability tests a few times per year, some relied on Google Analytics data which is absurd to me. Curious to find out more answers. How often do you do usability testing and what is your process?
