Best HIPAA-compliant issue tracker?

It's me again. The pariah of software users. The guy that has to comply with US HIPAA. In case someone posts PHI in a bug report.

I used to use JIRA, but since they ended their on-prem offering (well, technically, increased the price to $50,000), that's not a viable option.

So....what other options?
