2. Why is letting them die of disease superior to famine? You solve disease and then solve famine. Most places have neither.

I much prefer J.R.R. Tolkien: "Many that live deserve death. And some that die deserve life. Can you give it to them? Then do not be too eager to deal out death in judgement."
If the USA was even mildly smart, it would send veterans to visit the Killing Fields of Cambodia. That is what happens when good people do nothing.
