Admiral Cloudberg's medium posts are always incredible, but this one really struck a cord and made me contemplate how amazing some of the institutions in the US Government truly are.
It's all too easy to look at the SEC's absentee mishandling of crypto or whatever and take a dim view of US institutions. But all the way back in 1991, the NTSB was able to look at this accident, with a glaring first order cause of human error on the part of the controller, and recognize that it was actually a systems problem wherein any controller could have made the same mistake. Having agencies take actions like this, instead of a more typical CYA throw-the-individual-under-the-bus approach is something we should all appreciate.
