What was a pivotal moment for you? Do you believe it can be recreated? What advice would you give someone whom finds their situation lacking?

Ask yourself what bothers you and how you can change it.

For me, quitting my phd was one of the pivotal moments. I realized that a) it wouldn't get me where I wanted and b) the work environment wasn't fun at all.

C) when it's not appreciated, let alone, even known, how you do your work and what you do as your work.

D) change in management/ life that actually degrades your social status/ job's position

@OP: The pivotal comes automatically, when you realize you're stuck. If you think about "what's the proper moment to pivote" then you're not stuck.
