Enzo Clinic Labs breach, including test results [pdf] (enzoclinicallabs.com)

I just got a copy of this notice in the mail; looks like they lost the entire patient database including "patient names, dates of service, clinical test information, and, in some instances, Social Security numbers."

The company is a major provider of clinical tests in the North East.
