From recent reviews on iOS it sounds like all the email addresses a user submits are sent to the company the user wants to opt out of, regardless of how many have been shared by the user with that company. A dev responded that they'll add to the list of features the ability to select email addresses when submitting these requests. Sounds like it's a work in progress, and I'm optimistic.

I plan to try with a low-stakes email address I don't use much and have been manually decoupling from online services already. That's one way to find out how much you can trust this, but it'll take time and attention.
