Poll: How Does Your Company Deal with Mistakes/Failure?

Well, given the disagreements about how failure is treated in the workplace, I thought it'd be interesting to see how different employers handle screw ups on the job.

So let's assume something massive goes wrong. Customer details get leaked, the site goes down for 24 hours, important tables get dropped from the database, or the whole IT network goes boom, stopping anyone from doing work for days.
