Much of what she outlines is a systemic problem with modern civilization.
The biggest issue, and I see this as a consultant to top engineering and C level leadership is that no one wants to make decisions. You can't be accountable for the decision you didn't make, or so their theory goes.
We have big projects, led by committee that become their own organism and then everyone just goes with the flow.
