What is an effective manager?

What are the top 3 reasons that someone does not become an effective manager?

Main influences are Peter Drucker and Mark Horstman.

An effective manager is one who helps the team to succeed (engaged and happy) and aligns that with meeting organizational objectives

Could be a number of factors here but my take is lack of knowledge and lack of teaching
