My Nexus 5 just completely reset itself

I woke up this morning to find that my stock Nexus 5 running Lollipop 5.1.1 had factory reset itself. Its flashlight was stuck on, airplane was mode on, auto-rotate was disabled. I'm not signed into my Google account on my phone anymore. The default "Create" folder which has the Docs, Sheets, etc, icons in it is instead called just "T".

Has anyone had something like this happen before? Are there logs or something I can investigate to try to figure out what caused this? I was thinking maybe this was related to the Leap Second issue today? Apparently Google is spreading out the extra second over all of today.
