I came across this on my macbook pro. Kept falling even after reboot. Restarting and installing in safe mode got it upgraded for me.
The big one here for web-devs has to be "Show Tabbing Order" feature in the Accessibility menu in the dev tools. Being able to see this should save a lot of trial and error.
