I turned off email notifications. I put on earbuds and listen to instrumental(sometimes ambient) music and focus.

I used Tracks for to quite a while. I liked that you can host it on your own machine. I have a problem with some hosted site for my todo's. I too am looking for something that does hierarchies.
I used Anonymous for a while, but then I switched to Consolas. I'm not sure if Anonymous is free or not. One site wants to charge you for it, and another has it for free(no, not the Pirate Bay!).
